Holt Avenue

Holt Avenue is made up of 15 apartment units and a community meeting space with a laundry facility and office. The supportive housing facility offers five one-bedroom apartments and ten two-bedroom apartments for individuals with long-term mental illness disability. The Holt Avenue property sits on five parcels of land totaling 1.28 acres, which are located on the south side of Holt Avenue between Sunbury Road and Brentnell Avenue.

Resident services

Supportive housing is permanent affordable housing rented to people with special needs who are linked to individualized social services. Supportive housing services enable residents with special needs to manage their illness and contribute to the surrounding community. Resident services offered at Holt Avenue include primary case management and mental health and chemical dependency counseling services; individual and group programming, such as goal setting, physical fitness, basic life skills, budgeting, addiction education, job preparation, family reconciliation and leisure activities; and linkage to medical, dental, legal, and employment services and entitlements. Services are provided by the Alcohol, Drug, and Mental Health Board of Franklin County system agencies.

Project management

CHN owns and manages Holt Avenue. An onsite resident manager provides general building and resident oversight. CHN will continue to protect its investment in this property through planned quality maintenance and attractive landscaping.
